Description
  • Demonstrates knowledge and skills necessary to provide services based on the physical, psychosocial, educational, safety, and other related criteria appropriate population in his/her assigned area.
  • Maintains required core competencies.
  • Complies with set Policies and Procedures (i.e. name tag, dress code, parking, smoking, etc.)
  • Maintains appropriate amounts of forms, materials, and supplies relative to patient census.
  • Completes assignments without incurring unauthorized overtime.
  • Does not abuse sick or personal days; maintains personal health and adheres to Absenteeism policy.
  • Assists with the collection of Medicare letters prior to patient discharge.
  • Scan documents into medical records as needed.
  • Call PCP offices to schedule hospital follow up appointments.
  • Notify Home Health/Hospice Agencies, skilled nursing facilities and PCP clinics if a patient expires or transfers out of the area.
  • Fax Face to Faces for Home Healthcare/Hospice agencies to patient's PCP office.

Licenses & Certifications

C-BLS
